1. Picks are an issue, but it's not just about picking 5 carries or all melee (which does happen a lot). Pubs never seem to understand how to comprise a lane. It's more than just carry + support = lane. If your safe lane has pl support him with a disabler or something as he already has a slow. But pubs seem to think any support is fine, they'll mix tide and pl which is far less than ideal and you'll lose vs lanes that setup stronger and know what they're doing.
2. Jungle in general. First they try to jungle heroes that are bad at it or way more valuable in lane like lycan and axe (axe pwns in lane, lycan is too weak to jungle at lvl 1 efficiently). The other issue is they think jungle means go afk and farm. They'll go doom or lifestealer and never leave until like level 10. Even if there's a great gank in the safe lane they never leave.
3. Probably the biggest issue though is general map awareness. They rotate very poorly. Your safe lane might be getting pushed but you could easily turn it around into a gank if a couple just tp or mid rotates but it doesn't happen no matter how many times you ping. You might be facing a roaming gank team and have to switch to more defensive until you catch up but people will still push way too far out solo. Not paying attention to missings. Pushing for a gank when mid is missing so it turns into a counter gank. Just general crappiness with not realizing when enemies are coming.

There's also the opposite situation, where there is no hero with an invis skill in the team, but everyone builds Shadow Blades.
Of course, the mistake here lies in the enemy team not picking any kind of detection until minute 30.
