it's not that item is bad, far from it, it just delays core items by a thousand.
you could get it on heroes that look to fight early and need mana/armor from it, like bristleback
but on most heroes there are other items more important to get in order to come online sooner.

5->6 armor
6->7 armor + can be casted on allies for opposite effect.
My suggestion is, make it replace the cuirras recepie.
You all are wrong. Pub players lack teamplay, you may use medllion on one enemy, but your teammates may decide to focus another one. This is why this item is bad for pubs.
i always make it on lycan. Great snowballing item for him. Mid-lycan with early medallion and vlads RAPES entire enemy team. Then I make AC (and never deso).

Very rarely see Medallion picked up in pubs (2nd least popular non-consumable, non-upgradable item on DB), and even in pro games only on Visage or when teams look for Rosh in the first 20 min. I think if it could be thought of as a ganking item, people, particularly core players, would be more interested in picking it up early. It is a -7 armor debuff, almost as much as a deso, albeit without the added damage. Still, on heroes who have expensive core items with little buildup, it seems like it would make sense. Heroes like Slark, Jugger, and TA could pick it up after one core item (such as a blink -> MoC -> Skadi on Slark, or MoM -> MoC -> Agh's Jugg) and it provides a great deal of damage amp.
Boring Math Stuff (I went overboard, mateys)
Level 11 Jugg with MoM dealt an average of 1000 damage against a 12-armor target with Omni when I tested it.
With Yasha as well, this rose slightly less than 100, giving us ~1100 damage.
Replace Yasha with Medallion, and this hits 1450. This is almost a 45% increase in damage from the first test, for 1200 gold.
When enemies have lower armor, the numbers become even more significant. At 7 armor, you get ~1150 - 1350 - 1750.
(I skilled stats over Blade Dance, to keep the numbers from changing too much)
I was thinking maybe Medallion could get a buff that was very obviously oriented towards finding kills with it, and it would make it not a joke of an item to most players.
My idea was that invis heroes could use medallion on a target without becoming visible, and the debuff would not appear on the status bar until that invis hero appeared on the map again. This, together with the pushing power that the last medallion buff gave, might bring it out of obscurity, maybe?
